Former patients of Gilbert Hospital and Florence Hospital at Anthem can get copies of their medical records — but not for long.

Both locations closed last year after their owner, New Vision Health, LLC,  filed for bankruptcy protection.

A court-appointed receiver now says that as part of the winding-down of the operation, former patients can get copies of their medical records.

Here’s the key part: people must request copies by June 23. After that, the plan is to destroy all medical records.

“We’re hopeful that the message is received by people and we would encourage them not to wait till the end,” said receiver Jeremiah Foster, with the firm Resolute. “This is the best we could get given the circumstances. This is a very complicated matter and from the beginning we were always fighting to basically preserve and protect medical records.”

Patients and other appropriate parties have to fill out a medical record request form. Links to forms for both hospitals, as well an email address and phone number, can be found here.

A Texas company announced last year it would re-open at least one of the hospitals. But medical records do not transfer to any new owner of the building.
